A consultation at a community alcohol and drug support service: a social worker discusses relapse, cravings, and recovery strategies with a client.

  1. Social Worker · turn 1

Hello, Mr Zhong. Thanks for coming in today. I’d like to have a chat with you and see how you’re doing these days. When we last spoke, you’d managed to stay sober for a couple of weeks. How have things been since then?

  2. Mr Zhong · turn 2

Hello, James. To be honest, I've started drinking again recently. My wife is very upset about it. She doesn't want to see all the effort I made end up being wasted. I'm quite disappointed in myself too, and feel like all my previous efforts were in vain.

  3. Social Worker · turn 3

I see. Try not to think of one setback as wiping out all the progress you’ve made. A few setbacks can happen during recovery, particularly in the initial stages. You might need to set yourself a more realistic goal — say, not drinking for at least three days a week to begin with — and gradually build from there.

  4. Mr Zhong · turn 4

I know I need to take it one step at a time, but the problem is, the more I stop myself from drinking, the more I crave it. Sometimes when the urge hits, all I can think about is alcohol. And when I'm sober, I always feel empty and scared inside, whereas being drunk actually feels a bit more comfortable.

  5. Social Worker · turn 5

What you’re describing sounds like cravings and withdrawal symptoms. They can occur because you’ve become dependent on alcohol, both physically and psychologically. To reduce that dependence, you’ll need to stay away from alcohol as much as possible and, at least for the time being, avoid situations where you’re likely to be tempted to drink. That may also mean keeping your distance from your old drinking buddies.

  6. Mr Zhong · turn 6

I know that all too well. Last week, a few old friends asked me out. I originally said no, but they insisted on dragging me out for a drink, saying, 'Just one drink, what are you afraid of?' In the end, one glass turned into a bottle, and we ended up drinking all night. When I woke up the next day, I had a splitting headache and my hands wouldn't stop trembling. I really can't go on like this.

  7. Social Worker · turn 7

That’s a good example of why triggers are so important in recovery. Your friends may not mean any harm, but being around people who are drinking can make it much harder for you to resist the urge. The shaking you mentioned may also be a withdrawal symptom. If you develop severe shaking, confusion, hallucinations or seizures when you stop drinking, you should seek urgent medical attention rather than trying to manage it on your own.

  8. Mr Zhong · turn 8

Actually, my biggest worry is that my body won't be able to handle it after quitting alcohol. Sometimes when I go a whole day without drinking, I can't sleep at night, my heart beats very fast, and I sweat. I used to think it was just a lack of willpower, but now I know it might be related to withdrawal reactions.

  9. Social Worker · turn 9

Exactly. Alcohol dependence isn’t simply a matter of willpower. Given the symptoms you’ve described, I’d like you to speak to your GP as well. They can assess whether you need medical supervision while reducing your alcohol intake. We can work alongside your doctor and help you with the social and psychological side of your recovery.

  10. Mr Zhong · turn 10

Then I'm willing to see a doctor. In fact, my wife has also been urging me to get treatment, but previously I felt it was very shameful to go into alcohol rehabilitation, as if only people who had completely lost control of themselves needed to go.

  11. Social Worker · turn 11

There’s no shame in getting support. In fact, recognising that you need help is an important part of recovery. I’m glad you’ve realised the importance of having a positive environment around you. That’s why you need a strong support network that can help and encourage you throughout the process, particularly when you’re dealing with cravings or feel that you might relapse.

  12. Mr Zhong · turn 12

I know. But to be honest, deep down I still feel very uneasy. Over the past few years, my life has been a complete mess, my job is unstable, and my relationship with my wife has been getting worse and worse; I simply can't see any hope. Sometimes I feel like a victim, as if every piece of bad luck falls on me.

  13. Social Worker · turn 13

I understand that things have been difficult, but getting caught up in self-pity is likely to lead you back into drinking again. Why not try to see things in a different way? You’ve come here voluntarily, you’re being honest about your relapse, and you’re taking steps to regain control of your life. That takes courage.

  14. Mr Zhong · turn 14

That's true. I should be more optimistic. As people often say, 'attitude determines everything.' If I go on thinking every day that I'm hopeless, then I probably really won't be able to change anything. I need to try to change my attitude.

  15. Social Worker · turn 15

That’s the spirit! But remember, being positive doesn’t mean pretending everything is fine. It means recognising that recovery takes time and knowing what to do when things get difficult. One practical thing we can do is identify your triggers and make a relapse-prevention plan.

  16. Mr Zhong · turn 16

How do we do that? I now know that going out drinking with those old friends is a big issue, but sometimes when I'm stressed, I suddenly get a strong urge to drink even when I'm at home alone.

  17. Social Worker · turn 17

That’s where a plan can help. We’ll identify situations, emotions and people that trigger your cravings, and then work out alternatives. For example, instead of reaching for a drink when you’re stressed, you might call your wife, go for a walk, attend a support-group meeting or contact our service. The important thing is to have several options ready before the craving hits.

  18. Mr Zhong · turn 18

That sounds much more practical than relying purely on willpower. My wife would probably support me too. It's just that I don't want to rely on her for everything; she's already been exhausted enough by all the turmoil I've put her through lately.

  19. Social Worker · turn 19

That’s understandable, and your wife shouldn’t have to carry the whole burden either. We can discuss professional counselling, peer support and local rehabilitation programs so that you have a broader support network. If your wife would like some support herself, there are also services available for family members affected by someone else’s drinking.

  20. Mr Zhong · turn 20

Okay, then I'm willing to give it a try. I used to think that quitting drinking was just about gritting my teeth and toughing it out on my own, but now it seems I really do need help from others.

  21. Social Worker · turn 21

Absolutely. Recovery isn’t something you have to do alone. We’ll take it one step at a time. I’d also like to arrange another appointment in two weeks so we can review how you’re going, particularly your alcohol intake, cravings and any withdrawal symptoms.

  22. Mr Zhong · turn 22

Alright. I really want to stick with it this time. Even if there are more setbacks along the way, I don't want to just give up as soon as I fail like I used to.

  23. Social Worker · turn 23

That’s a much healthier way of looking at it. Remember, you’re not in this alone. Give us a call or come back anytime you need support. And if you’re worried that you’re about to relapse, it’s better to reach out early rather than wait until things get out of hand.

  24. Mr Zhong · turn 24

Thank you so much. I will do my best to get my life back on track. I'll also have a good talk with my wife, and make an appointment with a GP as soon as possible to address the withdrawal symptoms.

  25. Social Worker · turn 25

Good. We’ll work through it together. I’ll give you the details of the support group and rehabilitation service before you leave today.

  26. Mr Zhong · turn 26

Alright, thank you. I feel much more at ease after our talk today.
