   Good morning, John. I’ve reviewed the blood sugar readings you’ve been recording at home, as well as your latest blood test results. Overall, your diabetes is reasonably stable, although your blood glucose is still running a little higher than I’d like.

   Not at this stage. I don’t think you need insulin yet. What I’d like to do first is increase the dosage of your oral diabetes medication and see whether that brings your blood glucose back into the target range.

   Diabetes can change over time, so your treatment sometimes needs to be adjusted. There’s another issue I’d like us to keep an eye on as well. You’re 57, you have diabetes, and you smoke. Those are all significant risk factors for cardiovascular disease.

   Not necessarily, and I don’t want you to assume that something is wrong. However, given your risk factors, I’d rather investigate it properly. I’m going to arrange a stress echocardiogram and an angiogram to check how well your heart is functioning and whether there are any significant blockages in your arteries.

   I understand it’s inconvenient, but I do think they’re important. Diabetes can increase your risk of cardiovascular complications, and smoking adds to that risk. If we identify a problem early, we have a much better chance of managing it before it becomes more serious.
